For the day of your visit:
- Please bring your Insurance information and ID.
- We need an legal guardian to accompany the child if he or she is less than 18 years. If someone other than legal guardian is coming along with the child, we will need a signed document from parents/legal guardian with a photo ID and phone number of the parent/guardian.
- Co-pays and deductibles are due at the time of appointment.
- We look forward to seeing you at your upcoming appointment.
- Please right down and bring your questions
- If you have done some labs - please bring it along with you. If you don't have access, please try and bring the name of the lab, where the blood test or imaging studies have been done, so that we can get access to them
- Part of endocrine exam will be examining the pubertal status of your child. This is enable us to access the growth and plan appropriate labs
